I/DD + Partnering with Telehealth to Reduce ER Utilization by Persons with I/DD
Persons with I/DD are challenged by a lack of training in the medical profession often resulting in an over-reliance on emergency room and urgent care settings for everyday medical and clinical concerns. Over the past year, Community Based Care has partnered with StationMD to provide telehealth services in its 14 ICF I/DD homes in Florida. The implementation of StationMD’s services required facilitative policies, change leadership, and continuous quality improvement efforts. In our presentation, we will describe the services provided, our implementation process, and the results – where over 9 / 10 individuals with a presenting medical or behavioral concern have been seen in the home, thus reducing traumatic and expensive trips to hospital and urgent care facilities.

During this session, you will make the connection between advocacy and what happens in Raleigh and Washington. NC Providers Council’s contract lobbyist will give an update on the legislative and political landscape as he explains what to expect during the long legislative session starting in January 2025. You will discover the dos and don’ts of advocacy and how to make your efforts more productive through a review of effective strategies and approaches in outreach. You will walk away with effective story-telling tips that help garner support for your issues. Join us for this advocacy-in-action educational session.

This workshop explores the critical importance of cultural competence in mental health settings for individuals of the African Diaspora. Recognizing the rich diversity within this community, we delve into the complexities of cultural identity, historical trauma, and systemic barriers that shape mental health experiences. Through interactive discussions and case studies, participants will gain practical strategies for fostering culturally responsive care, including enhancing communication, addressing implicit biases, and integrating culturally relevant interventions. By embracing cultural competence as an integral aspect of mental health practice, we aim to empower clinicians to effectively support the unique needs and strengths of individuals from the African Diaspora, ultimately advancing equity and wellness within our communities.

Miscellaneous + NC HealthConnex - From mandate to mechanism for improved care
NC HealthConnex is the statewide Health Information Exchange. While mandated for providers to submit data, it is also a mechanism that providers can use to improve care for their clients. It is vital in the new Tailored Care Management with NC Medicaid, and for follow-up, medication reconciliation, and continuity of care regardless of payor. The HIE offers several value-added services to assist providers in improving outcomes for North Carolinians.

Julia Adams-Scheurich will review the highlights of the 2024 North Carolina General Assembly session. During this presentation, attendees will be provided a review of substantive bills that have passed, their impact on the healthcare system in North Carolina, and their intersection with federal regulatory changes. The presentation will include an opportunity to provide a “look forward” on what to expect in 2025 and an opportunity to answer questions from providers and advocates.

Miscellaneous + Supporting Best Practices: Improving Quality and Effectiveness of Services
People with SMI can and do live independently in their communities every day with the right services at the right time. Team-based, person-centered services provide essential supports. To be successful and effective, Community Support Teams require a culture of collaboration, coordination, and communication with each other and with the individual being supported. In this session, we will provide an overview of how Technical Assistance and Coaching from the Technical Assistance Collaborative (TAC) provides support to Community Support Teams to provide recovery-oriented, person-centered, effective supports. You will also hear from one provider’s experience around how working with TAC impacted their services and the individuals they serve.

I/DD + Revenue Cycle Management on the IDD Provider Agency Mission
IDD Services providers have a three-pronged mission – maximizing individual care quality, financial stability, and growth. The foundation of this is optimized individual care, without which the mission is pointless. We will discuss how Revenue Cycle Management processes contribute to the individual care objective. Revenue cycle management needs to contain procedures to ensure that billing is timely and complies with funder contracts and process requirements. Some providers can struggle with reimbursement, particularly with Managed Care. Thin margins in our industry magnify this impact. Appropriate up-front management of the Revenue Cycle can minimize payer denials, thereby improving payments and required “re-touches” to get claims paid, and minimizing write-offs. Interestingly, these same disciplines support care quality, particularly driven by authorization compliance, care plan integrity, and timely visit verification. We’ll discuss how this all weaves together, and the benefits derived in this regard by a robust, full-featured agency management system.

MH – SUD + Reentry - Tailored Care Management for Incarcerated Individuals with SMI
Explore the benefits of Tailored Care Management (TCM) for individuals with Serious Mental Illness (SMI) reentering their communities after incarceration. Learn about a new pilot reentry program providing a “hands-on” Care Management approach with all matters related to reentry, from obtaining vital documents, transportation, peer support, community resources, and enrolling in appropriate community-based mental health/substance use services. This new initiative serves individuals with SMI coming out of prisons and county detention. Explore the benefits of engaging individuals in TCM up to 90 days pre-release, 90 days post-release, and continuous ongoing care management. Program goals include reducing recidivism, obtaining housing, increasing stability, reinstating Medicaid/disability benefits, and enrolling in appropriate BH/medical services.

Miscellaneous + VOICSS® Music Therapy model: Tapping in and Gaining Tools for Self-Efficacy and Emotional Behavioral Health
In this evidence-based interactive presentation, you will learn about a unique music therapy model (VOICSS®) that has shown strong outcomes statewide in North Carolina. Using unique modalities of learning, a non-directive approach, and strong curriculum content, this model builds critical skills needed for gaining self-efficacy and emotional behavioral health. Increasing partnerships as a service delivery pilot, the expanding program has brought innovation to camps and day programs in the community and classrooms across the state.
